It's not a click, it's a proc.
The difference is, procs go off during combat, while the item is wielded (not just sitting in a bag).
So, you need to wield the item, enter combat, and at some random time after that, the proc will go off, and you'll port.
The proc is insta cast (I think), but the time to get it to proc is not. I've never spent much time on melee chars, so I'm not fully versed in proc mechanics, but I think base proc chances are 2-3 times per minute (so every 20-30 seconds, again, on average. It could proc the first swing, and three more times in a row, RNG you know!). Some procs themselves have a higher (or lower) base chance to proc (see the item descriptions here for example), and melee chars get AAs which make procs more likely to happen.
EDIT - I know you said "proc" in your post, but based on your other post in the item thread, you said "it didn't proc when you clicked it", so I'm pretty sure you're conflating clicks and procs. They are two different things.
