I know shields are "armor" but since they go in an off-hand slot does toughness affect them?
Yes.
Toughness: "Increases your armor value from items by 2/4/6/8/10%"
Just like a Druid would benefit from Thick Hide or simply shifting to Bear form while wearing that +120 armor ring from the Tanaris turtle escort quest.

I was gonna mention that too: not just armor but shields, rings, even your weapon if it has armor on it is affected.
