Now if I was to guess to get the Pitchfork you'd need the:

Dimitre and I got the pitchfork by:
-We both joined a party, and met in San d'Oria
-We both traded a ginger cookie to an NPC at the start of the day, I turned into a ghost and he turned into a skeleton
-We both ran under a halloween "tree" decoration thing and each received a pitchfork.
At the time, there was no one at the decoration. Normally, there are like 800 people underneath it. If it doesn't work, it may be because there are a lot of people under it.

Yes that is correct.
Now for the Pitchfork+1 you need to do this:
Get 2 people from your nation.
Go to another nation. Equip your Fork (NQ)and trade a sweet.
You will both change into the same costume.
Now run under the deco and get your new +1.
